    Who it's for

    The Wesley Care Finder program is designed to support older people who:

      Face challenges communicating due to language or literacy barriers

      Struggle to understand information or make decisions on their own
    • Check or Tick Icon | Apricot
      Are hesitant to engage with aged care providers or government services

      Are in unsafe situations if they don’t receive proper care

      Are eligible for government-funded aged care services

      Live in an eligible local government area

      Have no carer or support person to assist them or don’t feel comfortable relying on their carer for help

    How to access support

    If you or someone you’re assisting meets the eligibility criteria, Wesley Care Finder can provide free, comprehensive support. Steps to Access Wesley Care Finder Support:

    Ensure Eligibility

    Confirm that the older person meets the program’s eligibility criteria, including the requirement to live in an eligible local government area and be eligible for government-funded aged care.

    The older person must give their consent before any personal information is shared with the care finder service.
