Compassionate support to help older people access aged care services.

Wesley Care finder program provides support for older people to access aged care services.  The care finder service is not available for everyone. It is specifically for vulnerable older people who need intensive support to access aged care and other local services Before applying, please check the criteria below for eligibility, services we can provide and area we funded for.

Who it's for

  • Seniors over the age of 65 (50 if Aboriginal and/or Torres Strait Islander)
  • Face challenges communicating due to language or literacy barriers
  • Struggle to understand information or make decisions on their own
  • Are hesitant to engage with aged care providers or government services
  • Are in unsafe situations if they don’t receive proper care
  • Are eligible for government-funded aged care services
  • Live in an eligible local government area
  • Have no carer or support person to assist them or don’t feel comfortable relying on their carer for help

What we do not cover

The program cannot help with:

  • support with hoarding and squalor
  • assistance outside the Health Districts listed
  • crisis support
  • clients who don’t meet the eligibility criteria
  • direct provision of housing or accommodation
  • individuals who have the means or capability to resolve their situation without intensive support
  • clients who are already being case managed by another service

If you or someone you’re assisting meets the eligibility criteria, Wesley Care Finder can provide free, comprehensive support. Steps to Access Wesley Care Finder Support:

Ensure eligibility

Confirm that the older person meets the program’s eligibility criteria, including the requirement to live in an eligible local government area and be eligible for government-funded aged care.

The older person must give their consent before any personal information is shared with the care finder service.
